How do you enroll in government health insurance?

How do you access the Patient Portal?

Our community outreach team can guide or assist you in registering for the Patient Portal. They will send you an email with instructions to create a username and password. Once your patient portal account is created, you will be able to access your visit summary and prescribed medications, among other benefits. To access the Patient Portal, click on the link located at the top of the page. If you have trouble accessing the Patient Portal, call or email during business hours at (787) 745-0340 or at the Patient Portal.

What happens if you need a specialist?

If your primary care physician refers you to a specialist, our coordinated care staff will assist you in scheduling appointments with specialists or other services. They will arrange transportation to medical appointments and ensure that you have all necessary prior approvals, pre-authorizations, and referrals when needed.

What insurances do we accept?
