Discount Rate (Sliding Fee) Refers to the discount applicable to the services offered by the SANOS Corporation based on income and family unit.
The Discount Program is a benefit provided by SANOS Corporation as a federally qualified health center (FQHC) to all interested patients classified up to 200% of the poverty level according to the federal poverty guidelines published annually by the U.S. Department of Health and Human Services.
Anyone receiving services from the SANOS Corporation may qualify for this program. Admission will be based solely on income and family size.
Eligibility:
- Individuals with or without health insurance
- Insured persons (high deductible, limited or no covered services)
The discount granted through this program is based on income and family size. A 20%, 40%, 60%, 80%, or full discount may be granted depending on your socioeconomic assessment.
The patient is responsible for completing the Discount Program Application annually.
Special Populations:
- Public Housing Residents with Mobile Medical Unit
- Specialized services for homeless people
Special Projects: Attention to vulnerable populations, focusing on social determinants.
